Patient Information:

Please call to schedule appointments at 410-266-6035 during normal business hours: Monday through Thursday 8:00 AM – 4:30 PM  |   Friday 8:00 AM – 3:00 PM. We welcome new patients to our practice. Any patient absent from the practice for 3 years or more will also need these forms. If you have the above forms completed, please arrive 15 minutes early so we may register you in our computer. You may either download the forms or request they be mailed.

Please bring to your appointment:

  • All patients are seen on an appointment basis. Please feel free to call us prior to your appointment to check for any delays. As a courtesy to other scheduled patients, our office adheres to a late policy of 15 minutes. If a patient arrives more than 15 minutes late for her scheduled appointment, she will be asked to reschedule. We require at least 24 hours notice for any cancellation. If an appointment is not cancelled, the patient will be charged a $25.00 “no show” fee. If you believe you have been charged a “no show” fee in error, then you have 30 days in which you can dispute this charge. Please note the $25.00 “no show” fee must be paid before your next visit.

    You need to schedule appointments for injections (such as Gardasil®, Depo-Provera, or Depo-Lupron®).

    You do not need to schedule appointments for blood procurement but please call ahead to confirm.

For your convenience we accept cash, personal checks, master card and VISA. We accept most major insurance plans.

Co-payments must be made prior to the patient being seen. This is required in terms of your contract with your insurance company. Any amounts that are applied to the patients’ deductible are due and payable prior to the patients’ next visit or within 30 days after we receive notice from your insurance company, whichever comes first. Patients must present appropriate insurance information or the visit must be paid-in-full at the time of the appointment. If your card does not have the appropriate information listed, then you will be responsible for the visit.

For IUDs and Nexplanon®, we request payment at the time of service. These procedures also require insurance pre-authorization. Please contact your insurance company to verify your benefits. If the service is covered, we will bill the insurance company and reimburse you the cost of the device once payment is received from the insurer.

If you have any questions regarding your health or want to make an appointment, please feel free to contact us Monday thru Thursday 8:00 am to 4:30 pm at 410-266-6035, and Friday between 8:00 am and 3:00 pm.

  • Dr. Hardy is generally in the office Monday through Wednesday between 9:00 am and 4:30 pm, Thursdays and Fridays between 9:00 am and 12:00 pm. The schedule may sometimes change due to surgical procedures.
  • Linda is generally in the office on Tuesdays and Thursdays from 8:00 am to 4:30 pm.
  • For After Hours, if you have an emergency that requires immediate attention and our office is not open your call will then be transferred to the answering service at 443-481-1000. You will then be directed to the on call physician for assistance.
  • Moving? New Job? Or just want a medical review of the last few years? Ask us for your medical records. Please give us 7 business days to process a Medical Release form request.

    For permanent records transfer, there is a fee of  $20.09 + $0.73 cents per page and postage for copying and administrative costs. Via your preference, a statement will be mailed or emailed to you prior to the release of your records.
